What to Bring to an Appointment

Gather the following items to bring with you to your passport appointment:

Passport Application

Pick up applications at the Quarryville Library or download the form by clicking this link.

Complete the form by filling in all the required information. Write legibly using black ink. If you make a mistake, you will need to fill out a new page. Do not use white out or any other product that covers portions of your application. Do not sign the application.

Proof of Citizenship

Birth Certificate – This must be the original, state issued document with a raised seal. It should say “Birth Certificate” or “Certification of Birth” and be issued from the “Department of Vital Statistics.” Your birth certificate must list both parent’s names. Your birth certificate will be sent with the passport application and returned with your newly issued passport. (See the bottom of this page for a sample of an acceptable Pennsylvania birth certificate.)

OR

Certificate of Naturalization – This must be the original document with a raised seal. This will be sent with the passport application and returned with your newly issued passport.

OR

Previously valid US Passport that is not eligible for renewal by mail. This will be sent with the passport application and returned with your newly issued passport.

Valid Photo Identification with Signature

Your driver’s license is the best choice. If you are from out of state you must have a second form of identification. Picture ID is preferred, and the more information on your identification, the better.

Passport Photos

1 passport photo with very specific specifications must be submitted. (Please refer to U.S. Department of State Website.) You can have your passport photo taken at the Quarryville Library for $15 per person.

Payment

Personal checks are the best method of payment when applying for your passport, but money orders are also accepted by the U.S. Department of State. At least two checks will be necessary: one to the U.S. Department of State and one to the Quarryville Library. However if you are paying for multiple members of your family to apply for passports, each application must be accompanied by its own individual check.

Amounts will vary depending on your situation, so please wait until your appointment to make out checks.

Please note: The library will also accept cash, check, or credit card for the execution fee.
